05/22/2001 Resolutions 2001-131 I RES 0 L UTI 0 N 2001-131 WHEREAS, on November 21, 2000, by Resolution 2000-355, the City Council for the City of Grand Island awarded the bid for the construction between US Highway 34 and Stolley Park Road to the Diamond Engineering Company of Grand Island, Nebraska; and WHEREAS, it has been determined that modifications to the work to be performed by the Diamond Engineering Company are necessary; and WHEREAS, such modifications have been incorporated into Change Order No.1; and WHEREAS, the result of such modifications for this project will increase the contract amount by $82,305.11. NOW, THEREFORE, BE IT RESOLVED BY THE MAYOR AND COUNCIL OF THE CITY OF GRAND ISLAND, NEBRASKA, that the Mayor be, and hereby is, authorized and directed to execute Change Order No.1, between the City of Grand Island and the Diamond Engineering Company to provide the following modifications: I 22 8" concrete was used instead of 6" at the Oak Grove Inn 131.3 S.Y. @ $25.55 Deduct $3,354.72 parking lot. (See Item No. 313) 60 Jack and bore 54" RCP at station 106+56 instead of No $0.00 shoring. This will be done at no increase in contract Change amount, and will be paid for as Shoring for 54" Storm Sewer Pipe. 137 Increase the planned quantity of pile lengths for the 380 L.F. @ $30.65 Add $11,647.00 Wood River Bridge. The additional length was needed to obtain designed bearing capacity. 266 Millings used instead of temporary gravel surfacing 150 C.Y. @ $6.85 Deduct $1,027.50 272 Millings used instead of 6" temporary asphalt. 1667 S.Y. @$24.80 Deduct $41,341.60 273 Millings used instead of 5" temporary pavement 1200 S.Y. @ $24.80 Deduct $29,760.00 274 Millings used instead of temporary gravel surfacing. 350 C.Y. @ $8.25 Deduct $2,887.50 New Bid Item No. 310 Connect new water services to existing services at curb 25@$550.00 Each Add $13,750.00 stop 311 Extend the arm of the 36" sluice gate that is to be Add $100.00 installed in the head wall at the Wood River. This will make the height of the both sluice gates the same so they will be easier to operate. 312 Increase the size of the sanitary sewer that goes Add $176.00 through storm sewer manhole #5 from 4" to 6". This I sanitary sewer was bid as a 4" line, during construction it was discovered that the existing sewer was 6". 313 8" concrete pavement for parking lots. This was needed 131.3 S.Y. @$31.10 Add $4,083.43 at the Oak Grove Inn parking lot since the concrete that was removed was 8". (See Item No. 22) Approved as to Form T C(JC' May 18, 2001 . City Attorney I 314 Remove and re-install traffic detectors and pull boxes, Add $5,995.00 install new conduit and cable in the northwest quadrant of the intersection of Highway 34 and S. Locust. The existing detectors will be damaged during construction and the pull boxes conflict with the proposed storm sewer improvements. 315 Asphalt millings to be used as temporary surfacing in 5350 S.Y. @ $8.00 Add $42,800.00 place of temporary asphalt, concrete, or gravel. (City supplied millings) 316 Asphalt millings to be used as temporary surfacing in 5350 S.Y. @ $14.50 Add $77,575.00 place of temporary asphalt, concrete, or gravel.
